Thank you for visiting Lafayette. We don't want to push you in any way but we are also anxious to get to know you better. If you are willing, please fill out the form below with only as much information as you are comfortable sharing.

If there are others in your family, please have EACH family member fill out this form individually (or fill it out for them), it helps us get to know everyone better. The information will only be shared with church leaders unless you give us permission to put you in our Breeze Online Directory (you will have that option on the form). Likewise, you will have access to directory information of member names and their pictures so you can get to know us better too.

Only your name, picture, address, and phone number will be shared in the directory (and you can choose to keep those private as well). Any other information you share will only be shared with elders, ministers, and other necessary ministry leaders - including your year of birth.

After you get to know us, if you feel like you would want to talk more to someone about becoming a part of the family at Lafayette by placing membership, please contact one of the elders or ministers, or send an email to the church office - office@lafayettechurch.org.
